BACKGROUND
The FUTUREPLANS project is focused on helping youth and adults in Appalachia discover their best self and growing the economic prosperity of the region. Project REACH is part of the Future Plans initiative.
The primary purpose of Project REACH (Recovery to Employment via Assessment and Coaching, Holistically) is to strengthen the recovery ecosystem in each of the three service counties (Jackson, Pike, Scioto Counties, Ohio) by facilitating workforce entry or re-entry for those in Substance Use Disorder (SUD) recovery who are also navigating the legal/justice system. The goal of the project is to create a smoother pathway between recovery/incarceration and the workforce community by providing transitional workforce support to both participants and employers.

PURPOSE
The purpose of Project REACH Holistic Care Coordinator is to meet the adult population where they are by serving as a coordinator to outside referrals with a focus on the individuals overall wellbeing. The Project REACH Holistic Care Coordinator will help the adult client grow through a hands-on process of utilizing FP assessment and local partner resources, to improve their overall career path and financial sufficiency (with growth measured in yearly salary increase). This position will assist the Adult Program Team in community efforts to help all adult populations. The expectation of this position is to assist the three pilot projects Career Mentors clients (after coaching) in assigned counties, know, and build relationships with local systems which impact adults requiring additional support and participate in building a holistic approach to care for each client on an individualized basis..
DUTIES
- Collaborate with local partners to utilize FP career assessment data to help the adult population grow everyone’s career path towards an in-demand job or higher financial rewards (self-sufficiency).
- Maintain data in the FP dashboard and/or other client data monitoring platforms to monitor career readiness status of all clients (as assigned).
- Collaborate with the Adult Program team, the local libraries, and Ohio Means Jobs office to develop referral opportunities for clients.
- Coordinate the transition of follow up information of associated client’s information with local agencies.
- Assist partnering agencies on the use of FP data for clients when developing an educational plan and/or career plan to help the client balance their overall wellbeing.
- Collaborate with FP Community Outreach Partners and Adult Project Managers as needed.
- Serve as program representative on assigned community committees such as drug coalition, recovery, and court initiatives. This can be in collaboration with The Adult Program Team.
- Identify local success stories for the purpose of developing video testimonials and local press coverage.
- Be knowledgeable and maintain relationships with local community colleges and workforce training programs such as apprenticeships for client referrals.
- Maintain positive community relationships as a representative of FP.
- Any other duties and responsibilities as assigned by Project REACH Manager and Director of Adult Initiatives.
REQUIREMENTS
- Demonstrate the ability to communicate and work effectively with a diverse range of people and formal organizations.
- Ability to build effective relationships and consensus through effective collaboration.
- Demonstrate initiative and ability to think in an interdisciplinary team.
- Must have a reliable automobile, valid driver’s license and minimum state required liability auto insurance.
- Complete Ohio’s Criminal Background Check.
Job Type
- Full-time
- Schedule, Monday – Friday, including summer months.
- Some weekend and/or evening availability.
- Position requires travel as needed and as approved by Project REACH Manager
QUALIFICATIONS
- Preference given to those with experience working professionally as a Counselor, Program Coordinator and/or Social Worker.
- Preference given to those with extensive experience and knowledge of working in the community with the local population. This experience could be as a community volunteer, school volunteer, civic leader, or other community involvement.
- Must have experience working one on one with individuals.
- Must be willing to complete required training for position within the first 90days of hire.
- Must be proficient in using technology such as Microsoft office or Google docs
These qualifications are guidelines. Other combinations of education and experience which could provide the necessary knowledge, skills, and abilities to perform the job will be considered for potential to hire.
LOCATION
This position is a field position and offers the opportunity to work from home as well as from partner locations.
